Chronic back pain is commonly described as deep, aching, dull or burning pain in one area of the back or traveling down the legs. Patients may experience numbness, tingling, burning, or a pins-and-needles type sensation in the legs. Regular daily activities may prove difficult or impossible for the chronic back pain patient. They may find it difficult or unbearable to work, for example, even when the job does not require manual labor. Chronic back pain tends to last a long time, and is not relieved by standard types of medical management. It may result from a previous injury long since healed, or it may have an ongoing cause, such as nerve damage or arthritis.
Acute back pain is commonly described as a very sharp pain or a dull ache, usually felt deep in the lowerpart of the back, and can be more severe in one area, such as the right side, left side, center, or the lower part of the back. Acute pain can be intermittent, but is usually constant, only ranging in severity.
Sometimes, acute back pain can be caused by injury or trauma to the back, but just as often has no known cause. Patients with acute back pain, even when it's severe, will typically improve or completely recover within six to eight weeks.
Approximately half of all back pain patients have acute pain caused by trauma. A contusion, torn muscle, or strained joint resulting from a back injury can cause acute pain. Patients with any of these conditions typically exhibit pain, muscle spasms, and decreased functional activities. Treatment of acute back pain is short-term and usually successful. With physical therapy, follow-up treatment, and prevention practices, these patients typically return to full functionality in a few weeks. Occasionally, these patients will re-injure themselves and have to return for a short course of treatment. Patients with acute pain occurring more than three times in one year or who experience longer-lasting episodes of back pain that significantly interfere with functional activities (e.g., sleeping, sitting, standing, walking, bending, riding in or driving a car) tend to develop a chronic condition.
Fibromyalgia is a chronic illness that causes immense physical pain and debilitating fatigue. Classified as a syndrome, fibromyalgia is not a disease. Instead it is a condition that causes many different symptoms. These symptoms affect all systems in the body. Fibromyalgia plagues up to 5% of the population, with 6 million sufferers in the United States alone. The name fibromyalgia comes from "fibro" meaning fibrous tissue (such as tendons and ligaments), "my" meaning muscles, and "algia" meaning pain.
The Pain of It All
Fibromyalgia's main symptom is widespread musculoskeletal pain. This pain affects the ligaments, tendons, and muscles throughout the body. Ligaments are made of tough tissues which attach to the bones in your body. Tendons help to attach muscles to your bones. Muscles allow you to stretch and flex various body parts.
Fibromyalgia patients often feel like they are aching all over; muscles feel sore, stiff, and overworked. Muscle twitches are also common in fibromyalgia, adding to the pain of the illness. Sometimes muscles can even feel like they are burning.
A Chronic Disorder
Fibromyalgia is a chronic disorder, which means that pain can continue for months and even years. Unfortunately, fibromyalgia treatment is often difficult to come by and many people must suffer in silence. However, fibromyalgia pain generally does not worsen over time, and it doesn't cause inflammation of the internal organs or muscles.

Leg pain can be due to a muscle cramp (also called a charley horse). Common causes of cramps include:
  • Dehydration or low amounts of potassium, sodium, calcium, or magnesium in the blood
  • Medications such as:

    • Statins, which lower cholesterol and can cause muscle injury

  • Benign tumors or cysts of the femur or tibia (osteoid osteoma)
  • Drugs such as allopurinol and corticosteroids
  • Legg-Calve-Perthes disease -- poor blood flow to the hip that may stop or slow the normal growth of the leg
  • Malignant bone tumors (osteosarcoma, Ewing sarcoma)
  • Sciatic nerve pain (radiating pain down the leg) caused by a slipped disk in the back.
  • Slipped capital femoral epiphysis -- usually seen in boys and overweight children between 11 and 15 years old

There are many conditions that can cause chronic neck pain. The following information describes symptoms of some of the more common causes of chronic neck pain.

Neck pain that radiates down the arm

Pain that radiates down the arm, and possibly into the hands and fingers, is frequently caused by a cervical herniated disc or foraminal stenosis pinching a nerve in the neck. The pain may be accompanied by numbness or tingling in the arms and/or hands. The symptoms may start suddenly or develop over time.

Neck pain that is related to certain activities or positions

Pain that develops slowly (often over a number of years) and tends to occur during or after certain activities or neck positions is frequently caused by cervical foraminal stenosis. Usually, impingement of one nerve root on one side of the spine causes most of the symptoms.

The condition is caused by wear or aging related changes in the joints of the neck (facet joints) or at the margins of the discs. These changes may be diagnosed by either an MRI or a CT scan with a myelogram. As with a herniated disc, the mainstay of treatment is medical care (medicine, therapy, exercise, injections, etc.)

Neck pain that persists for more than a few months and may fluctuate

Neck pain that is often characterized by a low level of chronic pain that sometimes “flares” and gets worse, is made worse by certain positions or activities, and may be accompanied by arm pain, may indicate symptomatic cervical disc degeneration.

Neck pain that is worse in the morning and at the end of the day
Paradoxically, there are also patients who feel their worst when they first rise in the morning and at the end of the day. These people often feel best when they are moving their neck, and they often prefer warm, sunny days to cool, rainy or overcast days. Such symptoms parallel those experienced by patients with osteoarthritis of the weight-bearing joints (e.g., hips and knees). It is presumed that arthritic changes in the facet joints play a role in these people.

Axonal neuropathy is the more common "run of the mill" type neuropathy, while demyelinating neuropathy is less common and usually has a more specific pattern to it. The axon is the long nerve cell process that conducts electrical impulses to (sensory) or from (motor) the spinal cord. A nerve is like a cable consisting of numerous axons. A fair number of axons are surrounded by a segmented fatty insulation called myelin. This helps conduct electrical impulses faster. In axonal neuropathies the axons die or suffer injury. The longest axons (to the toes) suffer damage the earliest, in a form of axonal neuropathy called "peripheral neuropathy" or "polyneuropathy". Axonal neuropathy, if patchy and asymmetric, is often termed "mononeuritis multiplex". Diabetes, drugs, toxins, vasculitis, uremia, etc. are some of the more common causes of axonal neuropathy. Demyelinating neuropathy involves damage to the myelin with relative preservation of the axons. Some forms run in families, in which case the neuropathy is termed "Charcot-Marie-Tooth syndrome", or CMT, in which nerves are uniformly affected and slow in their function. Acquired or inflammatory forms of demyelinating neuropathy are more common, and fall into the broad categories of acute (Guillain Barre syndrome = GBS) and chronic (CIDP). Both of these, specially GBS, tend to be quite patchy, and weakness can be quite severe. GBS and AIDP have specific effective treatments. Some forms of chronic demyelination neuropathy have an association with some forms of lymphatic malignancy.

Chronic Pain can be disabling and depressing. I submit to you that each of us deals with our pain situation differently and no two peoples pain is exactly alike. Chronic back pain, chronic neck pain, Fibromyalgia, Neuropathy, RSD, Restless Leg Syndrome and many other diseases, afflict thousands upon thousands of us worldwide. I have been through numerous therapies in attempt to find ways to tolerate my chronic pain. Drug therapy, physical therapy, mental therapy; you name it we have tried it and we are still trying it today.
